چکیده

purpose: to report a case of posterior ischemic optic neuropathy (pion) following percutaneous nephrolithotomy (pcnl). patient and findings: a 57-year-old male who underwent pcnl because of renal stone was referred due to painless bilateral visual loss to the level of light perception immediately after the operation. the patient had history of diabetes mellitus, hyperlipidemia, and mild anemia. ophthalmic examinations were unremarkable and optic discs were pink and without any sign of swelling. visual fields were severely affected, but neuro-imaging was normal. within three months, visual acuity and visual fields improved dramatically and optic discs became mildly pale. the clinical course was typical for pion. conclusion: pion is a rare cause of severe visual loss following surgery. severe blood loss, hypotension, anemia, and position of the patient during surgery are the most important risk factors. this is the first report of pion following pcnl. urologists, ophthalmologists and anesthesiologist should be aware of this entity, and rare possibility should be discussed with the patient before surgery. preventive measures against pion should also be taken.

Anterior ischemic optic neuropathy following neck dissection.

BACKGROUND Ischemic optic neuropathy (ION) is a rare but devastating complication of surgery. It has traditionally been associated with intraoperative hypotension in patients with underlying arteriosclerosis. METHODS We present a case of ION following bilateral neck dissections in which there was minimal intraoperative hypotension and preservation of both internal jugular veins. The potential...

Pseudoaneurysm following percutaneous nephrolithotomy.

Delayed hemorrhage following percutaneous nephrolithotomy (PCNL) is secondary to pseudoaneurysm or arteriovenous fistula, which happens in less than 1% of PCNLs. Predisposing factors are: medial puncture instead of posterolateral puncture, arteriosclerosis, and hypertension. Definite diagnosis is made by angiography and embolization, as a definite treatment, can be done through the same angiogr...

A novel rodent model of posterior ischemic optic neuropathy.

OBJECTIVES To develop a reliable, reproducible rat model of posterior ischemic optic neuropathy (PION) and study the cellular responses in the optic nerve and retina. METHODS Posterior ischemic optic neuropathy was induced in adult rats by photochemically induced ischemia. Retinal and optic nerve vasculature was examined by fluorescein isothiocyanate–dextran extravasation. Tissue sectioning a...

MRI findings in Post-operative Bilateral Posterior Ischemic Optic Neuropathy.

A 62-year-old female complained of vision loss following multiple abdominal surgeries for mesenteric ischemia. The patient’s visual acuity was no light perception (NLP) in the right eye and hand motion (HM) at 1’ in the left eye. Both pupils were unreactive and no relative afferent pupillary defect was noted. Anterior segment and fundus examination were unremarkable. T1 and T2 weighted MRI imag...
